*You must meet criteria in order to schedule surgery in the Ambulatory Surgical Center. You will be evaluated at your consultation appointment with the surgeon of your choice in order to determine if there are any factors that would exclude you from this program.

CASH PAY FOR SLEEVE GASTRECTOMY
In order to proceed as a cash pay patient for the Sleeve Gastrectomy:
- Your BMI must be over 40 or 35-40 with associated medical problems
- At least 18 years of age
- You do not wish to proceed with insurance coverage for your procedure (many insurance companies currently do not cover the sleeve gastrectomy-info for 2009).
The Sleeve Gastrectomy surgical package is $14,900 and includes:
- Surgical Fees
- Hospital Fees
- Anesthesia Fees
- Program Fees
- The package also includes BLIS Complication Protection Insurance for Cash Patients at Hamilton Medical Center for 60 Days including:
- Cardiopulmonary (Heart and Lung problems)
- Thromboembolic (Blood clots)
- Bleeding
- Infection
- Leak
- Perforation
- Stenosis (Narrowing)
You will be responsible for obtaining a list of required pre-admission tests and x-rays determined by your surgeon prior to surgery. This testing will be at your own expense, however many if not all of these test can be covered by insurance through your primary care provider or a network provider. Your surgeon or anesthesiologist may also find it necessary for you to complete further tests based on the results of the above required testing. You will be notified if further testing will be required.
